Disabled Parking Permit Application Instructions:

1. If you are applying for your first permanent disability parking permit, you must complete the Application For Disabled Person’s Special Parking Permit with your social security number, notarized signature, your licensed physician’s original signature and license number. Stamped signatures are not accepted. Permanent permits are valid for two (2) years and there is no fee for the permit. Please mark the box for Placard in Section 1 of the form.
2. If you are renewing your permanent permit, you should complete the application form with your social security number and notarized signature.
3. If you presently have a disabled person or disabled veterans’ automobile license plate, simply present your automobile registration, the application form with your social security number, and have your signature notarized to receive one (1) parking permit.
4. If you are applying for or renewing a temporary permit, you must complete the application form with your social security number, your notarized signature, your licensed physician’s original signature, and license number (stamped signatures are not accepted). The temporary permits are valid for three (3) months and there is no fee.
5. If you have lost your permit or it has been stolen, you may obtain a replacement permit for no fee. You must complete the application with your social security number and notarized signature.
6. Any person who does not have a Veteran or Disabled Parking plate may obtain a second disabled parking permit for no fee. A new application form should be completed and the applicant’s signature must be notarized. The licensed physician’s original signature is not necessary for the second permit.
7. You may apply for parking permits in person or mail completed applications to our address.
8. Any organization which transports disabled persons as part of their services can obtain the parking permit. A statement from the director of the organization declaring that the organization transports disabled persons is required to obtain the parking permit.

1. To apply for a disabled permanent parking license plate, you must fill out the same application as the special permit listed above but check the box in Section 1 for License Plate. Fill out the Application For Disabled Person’s Special Parking Permit with your social security number, notarized signature, your licensed physician’s original signature, and license number. Stamped signatures are not accepted.
2. If an individual has a disabled permanent license plate they are allowed only one (1) parking permit.
3. The disabled permanent parking license plate renews every year in July, not the customer’s birth month.
4. The person acquiring the disabled permanent license plate must be the registered owner or one of the registered owners of the vehicle.
5. To obtain your disabled permanent license plate, bring the Application for Special Registration Plate for Disabled Person, the current license plate registered to the vehicle and the proof of insurance for the vehicle to the county clerk’s office.
6. To obtain the disabled permanent license plate the customer must fit one or more of the following requirements:
   
   1. Persons with disabilities which limit or impair the ability to walk (as declared by a licensed physician)
   2. Cannot walk two hundred (200) feet without stopping to rest
   3. Cannot walk without the use of or assistance from a brace, cane, crutch, another person, prosthetic device, wheelchair or other assisting device
   4. Are restricted by lung disease to the extent that the person’s forced respiratory and expiratory volume for one (1) second, when measured by spirometry, is less than one (1) liter, or the arterial oxygen tension is less than sixty (60) mm/hg on room air at rest
   5. Uses portable oxygen
   6. Has cardiac condition to the extent that the person’s functional limitations are classified in severity as Class III or Class IV according to standards set by the American Heart Association
   7. Are severely limited in their ability to walk due to an arthritic, neurological or orthopedic condition

  To obtain a copy of the Application For Disabled Person’s Special Parking Permit, click here.  Disabled Veteran’s License Plates: Veterans that have been declared at least 50% or more disabled by the Veteran’s Administration may obtain a Disabled Veteran’s License Plate

1. To obtain the disabled veteran’s license plate you must bring in proof from the Veteran’s Administration that you are 50% or more disabled, the current license plate on the vehicle and the insurance for the vehicle. Disabled Veteran License Plates renew every July, not the customer’s birth month. Personalized Disabled License Plates are available. Contact county clerk’s office for further details.
